Information for both applications

1) Make sure that you have both a distributor and
subscriber application.

2) Fill out the personal information on both applications.
Name, address (mailing address on the applications
please) city, state, zip code and phone number.

3) Place sponsor information in the correct position.

Information for subscription application only.

4) In step one choose a payment option.  The only options
are annually =$288 or monthly = $90/year and $18/month.
Annual equals $72 annual fee plus $18 monthly x 12 months.

5) In step two choose a monthly product purchase program
or MPPP.

6) In step three total the amount chosen in steps one and two.

7)Payment options. You can pay by check, money order, visa
or mastercard and check by fax.  If you choose the check by
fax option or the credit card option your applications can be
faxed in to expedite your entry into the program and expedite
the shipment of your starter kit.  If you are using credit card, make sure you get all the numbers transposed and sign the form where it says cardholder signature.  To use check by fax, make out a check to Celestial Harvest, just like you normally would, and fax it to us.  Note, please do not then send the original check to us.

FOR ALL PERSONS WHO CHOOSE THE MONTHLY OPTION
8) The final step is to fill out the automatic funds transfer
authorization.  If you are using this option, please enclose a
blank voided check for the account you would like debited and
sign the bottom of the form.  Note, you can use a C.C. for
monthly fees when combined with frequent shopper.

Information for distributor application.

9) The only additional information for this application is a
signature for applicant and if you would like some additional
sales aids fill out that portion.  If you are paying with a check
and getting sales aids you can include all these items in one
check.

The last step is to mail or fax the applications to the
home office.
